Original Medicare covers many health care costs and services; however, it does not cover things like coinsurance, co-payments, and deductibles. These are called “gaps”. Plan F also has high deductible options, which some companies may offer.  These high deductible plans pay the same benefits as Plan F and J after one has paid a calendar year $2000 deductible.  Benefits from high deductible Plans F will not begin until out-of-pocket expenses exceed $2000.

Plans K through N provide for different cost-sharing for items and services than Plans A through G.  With Plans K and L, once you reach your annual "out-of-pocket limit," the plan will then pay 100% of the Medicare co-payment, co-insurance and deductibles for the rest of the calendar year.  The out-of-pocket limit does not include charges for any provider that exceeds Medicare-approved amounts, known as "excess charges."  The annual out-f-pocket limits for these plans in 2010 are $4620 for Plan K, and $2310 for Plan L.

Deductible - Part A $1132 (appears twice in the benefit grid); Part B $162

With Plan M you pay 50% of your Part A Deductible ($1132 per benefit period), as well as the entire Part B deductible ($162 in 2011). Plan N pays all of your Part A Deductible, and you pay the entire Part B deductible ($162 in 2011) plus a $20 copayment for office visits and a $50 copayment for Emergency Room visits.

After the Medicare Supplement plan is mailed or delivered, you have a 30-day free look to examine the plan and to decide if you want to keep it.  If you return the plan within 30 days, all of your money will be refunded.

Eligibility and Enrollment

The following eligibility rules apply for Medicare Supplement applicants:

  • Enrolled in Medicare Part A and Part B
  • Both must be active at time of the plan effective date*
  • A resident of the state in which you are applying for coverage
  • Age 65 or older

Plans are available to people under age 65 and disabled in some states (Please consult the state-specific Availability and Eligibility Guides – link to plan availability PDF).

Open Enrollment is the first 6 months you are 65 or older and enrolled in Part B.  You may also qualify for a guaranteed issue plan if the following conditions apply:

  • Your Employer or Union coverage was terminated (voluntarily or involuntarily) within the past 63 days
  • Your Medicare Advantage coverage was involuntarily terminated within the past 63 days
  • Your existing Medicare supplement plan was terminated

Underwriting:  if you are enrolling outside an open enrollment period, the application will go through an underwriting process, and it is possible you could be denied. Conditions that will result in an application being denied include:

  • End Stage Renal Disease (ESRD)
  • Dialysis
  • Being admitted to a hospital within the past 90 days
  • Having a recommended treatment that have not been completed for one of the following:

    • Any hospital inpatient procedure
    • Organ transplant
    • Back or spine surgery
    • Joint replacement
    • Surgery for cancer
    • Heart surgery
    • Vascular surgery

New members receive a 12-month rate guarantee from your initial plan effective date. (You will not receive an additional rate guarantee when switching from one SecureHorizons Medicare Supplement plan to another.) Premium may increase each year at policy anniversary due to age (in most states) and inflation.

How to Get Signed Up

Enrolling in a SecureHorizons Medicare Supplement plan is easy. We’ll be happy to help you decide which plan is best for you.  Once you decide, you have 3 ways you can enroll:

1. Enroll online (coming October 1, 2010) - Here's how it works:

  • Follow the link below and enter your home ZIP code to find available plans in your area and view cost and benefit information.
  • Click the Enroll Now button for your selected plan to begin the online application process.

2. Enroll by phone - Call us toll-free at 1-866-323-1441, 9 AM – 11 PM Eastern to speak to a Medigap Advisor, and we’ll take your application right over the phone.

3. Fax or mail – Once you decide on a plan, contact us and we will mail or email you an application.  Then simply complete the application and fax it to us toll-free at 866-284-0082.  Mail the original copy of your application*, along with a check for the first month's premium or the completed EasyPay credit card form to:

Before submitting your application, be sure that you:

  1. Complete all forms in full and sign them.  Choose any effective date of coverage with the exception of the 29th, 30th, and 31st day of the month
  2. If you are not applying during an open enrollment period, be sure to answer all health questions.
  3. Include a check, money order or cashiers check for the first month’s premium.  All checks should be made payable to SecureHorizons.
  4. Complete the EasyPay form to authorize a bank draft or credit card charge for future payments.
