When you own a United of Omaha Insurance Company Medicare Supplement, you get the reputation of the parent company Mutual of Omaha, which has been providing financial products and services for over 100 years. United of Omaha is known to often offer the lowest monthly premium of any company available in the Medicare Supplement (Medigap) market place in 22 states where it is available.

Plan F also has high deductible options, which some companies may offer. These high deductible plans pay the same benefits as Plan F and J after one has paid a calendar year $2000 deductible. Benefits from high deductible Plans F will not begin until out-of-pocket expenses exceed $2000.
Plans K through N provide for different cost-sharing for items and services than Plans A through G. With Plans K and L, once you reach your annual "out-of-pocket limit," the plan will then pay 100% of the Medicare co-payment, co-insurance and deductibles for the rest of the calendar year. The out-of-pocket limit does not include charges for any provider that exceeds Medicare-approved amounts, known as "excess charges." The annual out-f-pocket limits for these plans in 2010 are $4620 for Plan K, and $2310 for Plan L.
Deductible - Part A $1132 (appears twice in the benefit grid); Part B $162
With Plan M you pay 50% of your Part A Deductible ($1132 per benefit period), as well as the entire Part B deductible ($162 in 2011). Plan N pays all of your Part A Deductible, and you pay the entire Part B deductible ($162 in 2011) plus a $20 copayment for office visits and a $50 copayment for Emergency Room visits.
After the Medicare Supplement plan is mailed or delivered, you have a 30-day free look to examine the plan and to decide if you want to keep it. If you return the plan within 30 days, all of your money will be refunded.

A Household Discount is available to:
- Individuals who reside together for at least one year (or are married), apply together for and are both issued United of Omaha policies
- Individuals who reside for at least one year with (or are married to) another Med supp policyholder who currently owns a Mutual of Omaha, United World, or United of Omaha Med supp policy. The discount only applies to the United of Omaha premium

Before submitting your application, be sure that you:
- Complete
all forms in full and sign them.
- Make
sure to use the correct application for your state.
- If you
are not applying during an open enrollment period, be sure to answer all
health questions.
- Include a check, money order or cashiers check in the amount of the premium for the mode selected, or the bank draft authorization, if premiums are to be drafted, submitted along with your application. All checks should be made payable to United of Omaha.

Underwriting
If you are outside of your open enrollment period, your application will be selectively underwritten. Be sure to answer all health questions. If you are applying for Plan N should not answer the comprehensive list of health questions. Plan N applicants in CO, FL, KS, ME, PA and SD are required to answer the End-Stage Renal Disease (ESRD) question. The answers to the health questions on the application will determine your eligibility for coverage. If any health questions are answered Yes, you will not be not eligible for coverage and will be declined.
Open Enrollment
To be eligible for Open Enrollment, you must be at least 64 ½ years of age (in most states) and be within six months of your enrollment in Medicare Part B. Applicants covered under Medicare Part B prior to age 65 are eligible for a six-month Open Enrollment period upon reaching age 65.
States with Under Age 65 Requirements:
Connecticut - Plan A is available.
Illinois - All plans are available. Coverage is guarantee issue if applied for within six months of Part B enrollment.
Kentucky - All plans are available. No guarantee issue. All applications are underwritten.
Louisiana - All plans are available. Coverage is guarantee issue if applied for within six months of Part B enrollment.
Missouri - All plans are available. Coverage is guarantee issue if applied for within six months of Part B enrollment.
North Carolina - Plan A available.
Guarantee Issue if applied for within six months of Part B enrollment.Oklahoma - Plan A is available. Coverage is guarantee issue if applied for within six months of Part B enrollment.
Oregon - All plans available.
Guarantee Issue if applied for within six months of Part B enrollment.Texas - Plan A is available. Coverage is guarantee issue if applied for within six months of Part B enrollment.
Wisconsin - Base policy and riders are available. Guarantee issue if within 6 months of Part B enrollment.
Application Dates
Here is when you can submit your application:
- Open Enrollment – Up to six months prior to the month you turn age 65.
- Underwritten Cases – Up to 60 days prior to your requested coverage effective date.
- Connecticut – Year-round open enrollment. Application can be submitted up to 60 days prior to your requested coverage effective date.
- West Virginia – Applications may be submitted up to 30 days prior to the month you turn age 65.
- Wisconsin – Applications may be submitted up to 90 days prior to the month you turn age 65.
Coverage Effective Dates
If you are between age 64 ½ and 65, your coverage will be effective the first of the month you turn 65. For all others, your plan will be effective on your application date or date of termination of other coverage, whichever is later.
