Medicare Special Needs Plans (SNPs) are a type of Medicare Advantage Plan that are specially designed to meet the needs of people with certain chronic diseases, those who live in a nursing home, or for people who are covered by both Medicaid and Medicare (also known as “dual-eligible.” For information on plans available to you, please select one of the following choices:
- I have a chronic illness
- I live in a nursing home
- I am Dual-eligible (have both Medicare and Medicaid)
Because they offer all health care services through a single plan, for example, a SNP may be designed to serve only people who have been diagnosed with congestive heart failure. The plan would include access to a network of providers who specialize in treating congestive heart failure, and it would feature clinical case management programs designed to serve the special needs of people with this condition. The plan’s formulary would be designed to cover the drugs that are usually used to treat congestive heart failure.
Medicare SNPs are approved by Medicare and run by private companies.
When you join a Medicare SNP, you get all of your Medicare hospital and medical health care services through that plan, including Medicare prescription drug coverage.
How Much Do Medicare SNPs Cost?
Medicare SNPs can be a very economical way to cover your medical expenses. With a SNP, you generally still pay your Medicare Part B premium, though in some cases the plan may cover part or all of the Part B premium as a plan benefit. You generally will also pay a Medicare SNP premium that helps cover costs of providing Part A and Part B benefits, prescription drug coverage, and any other extra benefits the plan offers.

When Can You Join a Medicare SNP?
