to Apply for Medicare Advantage Coverage
Applying for Medicare Advantage coverage is quick and easy. You can do it
online *, or just print out your application and mail it to us. Gather
the following information before you start the application in order to speed the
process:
- Address, phone number, social security number, and date of birth
- Name
of the plan you are applying for
- Your Medicare Claim Number and Effective
Date
- Payment Information to cover future premiums, if you are selecting
a plan including Part D prescription coverage

Choose a plan
Medicare Advantage
plans are offered by many different insurance companies. One recent report
showed over 1200 Medicare Advantage on the market, though over 700 of these
plans had less than 100 participants. Medigap Advisors recommends going

can check to see which plans are available in your area here. Remember,
most Medicare Advantage plans have no monthly premium. However, there

Online:
Once your Medicare Advantage options appear, you can click on “Show Details”
for more information on each of your options, can then click on “Enroll
Now” to apply online.

Choose Your Initial Payment Option
Only some Medicare
Advantage plans with Part D prescription drugs included have a premium.
If you choose one of these plans, you can have your monthly premium automatically
deducted from your Social Security check, or you can receive a bill each month.
If you choose
to have the payment deducted from your Social Security check, simply submit
your application and Anthem Blue Cross Blue Shield will contact you to set up
this deduction.
If you prefer to have your premium automatically deduct your premium and any
state-mandated fees from your checking account each month, simply enter your
account information on page two of your application. If you do not choose
this option, you will be sent a bill in the mail each month. Since failure
to pay your monthly invoice within 31 days of the due date could result in the
cancellation of your coverage, we recommend using the Electronic Funds Transfer
option.
If you qualify for extra help with your Medicare Prescription Drug coverage
costs, Medicare may cover all or some portion of your plan premium. In
this case you will only be responsible for the remaining premium.
Guarantee:
All plans come with a money-back guarantee. After
receiving your policy, you have 30 days to review it. If for any reason
you decide not to keep the policy, you will receive a full refund of your initial
premium.
