To Apply For Your Medicare Supplement Plan Applying

Open the application for the plan in your state. Click
here to select the plan in your state.
- Print it,
fill it out, and mail it to us at the address below. If you cannot print
it, just fill out our Application Request Form
and we will send you an application right away.
You
can choose to have your monthly payment automatically deducted from your checking
account each month. Simply download the Monthly Bank Draft/Electronic
Funds Transfer Authorization Form along with your application and include
it with the application. If you do not choose this option, you will be sent
a bill in the mail each month. Since failure to pay your monthly invoice
within 31 days of the due date could result in the cancellation of your coverage,
we recommend using the Electronic Funds Transfer option. Make
your check payable to the insurance company listed on youru application, and mail
it along with the application and EFT form (if applicable) to the address below. MediGap
